How do I request copies of my medical records?
You may visit Health Information Management, which is located on the second floor of St. Francis Health Center, from 8 a.m. to 4:30 p.m. Monday through Friday and complete an authorization to have your records released.
I do not have transportation, how can I get an authorization form?
A HealthPort representative would be happy to mail or fax an authorization form to you. You may request an authorization form by calling 785-295-8634 Monday through Friday from 8 a.m. to 4:30 p.m. You can then mail or fax (785-295-5300) the authorization back to St. Francis Health Center, Health Information Management, 1700 SW 7th St., Topeka, KS 66606.
How long does it take to get copies of my medical records?
The normal processing time for fulfilling a request for medical records is seven to 10 business days.
What is the cost for copies of medical records?
The cost for copies of medical records is according to the State of Kansas Department of Labor and is adjusted annually.
How do I pay for my medical records?
When your request for medical records is complete, you will receive an invoice from HealthPort by mail. The invoice will include the breakdown of the fees along with the address to send the payment to. You may also pay by credit card by filling out the appropriate field on your invoice. Once payment is received, your records will be mailed to you. If you would rather pick up your records, note your request on the authorization form.
